Where does WoundPlus™ come from?
WoundPlus™ is derived from naturally rich human placental tissues, donated by volunteer U.S. based donors after a live, healthy C-section delivery.

HydraTek® provides advanced preservation and an outstanding standard of safety, through its stringent donor screening, tissue testing, and strict quality control. All testing and safety standards meet or exceed the FDA requirements.
HydraTek® obtains its source tissues from healthy mothers who are under the care of a licensed OB/GYN physician at a partner facility. No maternal or fetal tissues are collected during the process. Intensive safety testing is used to screen both the donor and donated tissues before processing.
How is WoundPlus™
Applied?
Your provider will prepare the wound site by removing any non-viable tissue and excess fluid. |

When ready to use, they will remove the graft from the sterile pouch using dry, sterile gloves or forceps. |

Your provider will use forceps to apply the graft and achieve full contact with the wound, smoothing wrinkles as necessary. |

Your provider will apply non- adherent dressing, with multi- layer compression bandage. |
|
Your provider may re-apply as needed. Re-application at 7-14 days, if required, is typical. |

Your provider may prescribe anti- inflammatories or other medications as they see fit. |
Services Agreement: | How To Refer? |
This Services Agreement acknowledges a referral relationship between United Wound and Health Care Provider (HCP). The Business Associates agree to protect any personally identifiable or aggregate patient or other medical information defined as Protected Health Information ("PHI") in the Health Insurance Portability and Accountability Act of 1996 or its relevant regulations ("HIPAA") and otherwise meets the definition of Business Associate as defined in the HIPAA Privacy Standards (45 CFR Parts 160 and 164). United Wound Services are Billed directly to patients’ Traditional Medicare coverage. Your signature verifies receipt and understand that our services only cover wounds that are not related to the admitting diagnosis, and the accuracy of your contact information.

Step 1: Verification
Send United Wound the patient's Demographics and Pictures of Wounds
to:

Demographics should include Patient Name, DOB, Address, and Medicare Information
Step 2: Scheduling
Our Insurance Department will perform Medicare Part B verification. If patient has the appropriate coverage, our team will reach out to HCP or Clinician to confirm eligibility and schedule to complete initial evaluation within 48 hours
Step 3: Evaluation
United Wound Physician/NP completes initial evaluation. This includes recommendation for wound treatment, demonstration of wound care to PCG and Wound Care plan that is shared with HCP
within 48 hours of Evaluation.
Step 4: Continuation of Care
United Wound Clinicians complete Follow-Up Visits based on the Wound Care Plan,
Re-Evaluates patients every 7 days or as needed in the Wound Care Plan.
